8442 - ON-THE-JOB INJURY
Any employee incurring an injury while engaged in his/her performance of Corporation duties shall abide by the following procedures:
The injured employee shall report to the immediate supervisor and complete Form 8442 F2 before reporting to an approved physician. The only exception will be if the injury is of such a nature that it requires immediate emergency attention. In that case, treatment should be undertaken and a report of the accident made to the immediate supervisor at the earliest opportunity.
The appropriate form shall be completed by the appropriate administrator for injuries involving outside medical costs and lost time injuries with less than seven (7) days disability.
Except in emergencies, staff members are requested to use only hospitals and physicians approved by the Board and insurance carrier in the treatment of work-related injuries.

812-282-4037All necessary medical referrals shall be made only by physicians approved by the Board or insurance carrier. Upon completion of medical care by a specialist, the employee must first report back to the referring physician for release prior to returning to work.
Employee is expected to seek treatment at Business Health Floyd immediately when injury occurs.
If the Corporation does not believe the injury entitles the employee to receive worker compensation benefits, it shall so notify the employee and the Worker Compensation Board within thirty (30) days of the date at which the alleged disability begins, in accordance with the procedures prescribed by the Board. (I.C. 22-3-7)
