4160A - PHYSICAL EXAMINATION

1. Written evidence of good physical and mental health may be required periodically by the Corporation from a physician of the Corporation's choosing with the Corporation assuming the expense of such an examination.

2. On a voluntary basis, a staff member may complete an emergency medical authorization permit. (Form 4160 F1)
