5320 - IMMUNIZATION OF STUDENTS IN SCHOOL
Principals are to use the following guidelines in addition to those provided in AG 5111. - Admission to the Corporation.
Minimum Requirements for Entry (Effective beginning school year 2024-2025)

Hep B: The minimum age for the third dose of Hepatitis B is twenty-four (24) weeks of age.
DTap: Four (4) doses of DTaP/DTP/DT are acceptable if the 4th dose was administered on or after the child's fourth birthday.
Polio: Three (3) doses of polio vaccine are acceptable for all grade levels if the third dose was administered on or after the child's fourth birthday and at least six (6) months after the previous dose.
For students in grades kindergarten through 12, the final dose must be administered on or after the fourth birthday, and be administered at least six (6) months after the previous dose.
Varicella: Physician documentation of disease history, including month and year, is proof of immunity for children entering preschool through 12th grade. Parent report of disease history is not acceptable.
Tdap: There is no minimum interval from the last Td dose.
MCV4: Individuals who receive dose 1 on or after their sixteenth birthday only need one (1) dose of MCV4.
Hepatitis A: The minimum interval between the first and second dose is six (6) calendar months. Two (2) doses are required for all grade levels.
* For children who have delayed immunizations, please refer to the 2025 CDC "Catch-up Vaccination Schedule" to determine adequately immunizing doses. All minimum intervals and ages for each vaccination, as specified per the 2025 CDC guidelines, must be met for a dose to be valid. These guidelines can be found at https://www.cdc.gov/vaccines/hcp/imz-schedules/child-adolescent-age.html#table-1.
Statement of Immunization History
Each school shall require the parent of a student who has enrolled in the school to furnish, not later than the first day of school attendance, proof of the student's immunization status, either as a written document from the health care provider who administered the immunization or documentation provided from the State immunization data registry.
The statement must show, except for a student to whom I.C. 20-34-3-2 or I.C. 20-34-3-3 applies, that the student has been immunized as required under I.C. 20-34-4-2. The statement must include the student's date of birth and the date of each immunization.
A student may not be permitted to attend school beyond the first day of school without furnishing the documentation described above unless:
- the school gives the parent of the student a waiver; or
- the local health department or a health care provider determines that the student's immunization schedule has been delayed due to extreme circumstances and that the required immunizations will not be completed before the first day of school.
The waiver may not be granted for a period that exceeds twenty (20) school days. If the student's immunization schedule has been delayed due to extreme circumstances, the parent of the student shall furnish the written statement and a schedule, approved by a health care provider who is authorized to administer the immunizations or the local health department for the completion of the remainder of the immunizations.
Exemptions
If a physician certifies that a particular immunization required by I.C. 02-34-3 or I.C. 20-34-4 is or may be detrimental to a student's health, the requirements of I.C. 20-34-3 or I.C. 20-34-4 for that particular immunization is inapplicable for the student until the immunization is found no longer detrimental to the student's health.
Provisional Admission to School
A student may be admitted to school on a provisional basis if a health care provider or the local health department indicates that immunization of the student has been initiated and that the student is in the process of complying with all immunization requirements. A schedule of proposed immunizations must be on record at the school office.
Documents Accepted as Evidence of Immunization
The following documents will be accepted as evidence of a student's immunization history provided they comply with State requirements and contain the date when each immunization was administered:
a written document from the health care provider who administered the immunization
documentation provided from the State immunization data registry
Required Records
If a student transfers to another school, this record or a copy thereof shall be sent to the new school within twenty (20) days.
If a student enrolls in a State education institution, the student's immunization record shall be sent to the State educational institution.
If a student is enrolled in a State educational institution while still attending a secondary-level school, the student's immunization record shall be sent to the State educational institution.
Information concerning meningococcal disease shall be provided to students and parents at the beginning of the school year.
Materials concerning immunizations and immunization preventable diseases shall be provided to parents and guardians of students by the principal of each school, who shall obtain these materials from the State Department of Education. Posting the materials on the school building’s website shall satisfy the distribution requirement.
Information concerning the link between cancer and the human papillomavirus (HPV) infection, and that an immunization against the human papillomavirus (HPV) infection is available as prescribed by the State Department of Health shall be provided to the parent of each student who is entering grade 6.
Annual Review of Immunization Data in State Registry
The principal of each school shall submit complete immunization data for the students in their school using the Indiana State Department of Health's immunization data registry by the first Friday in February each year, notify the Superintendent that they have completed this task, and provide the Superintendent with a copy of the immunization data submitted. The Superintendent shall review all of the immunization data for the School Corporation in the state immunization registry to ensure it is complete not later than the first Friday in February each year.
Records Available for Inspection
The principal shall make immunization records available for inspection by authorized representatives of the State Department of Health.
