5200.01 - ABSENCES RELATED TO TREATMENT OF AUTISM SPECTRUM DISORDER PROCEDURES

The Florida Department of Education requires school districts to adopt a policy and establish procedures, including timeframes, for parent/legal guardians to request excused absences for scheduled appointments for treatment of Autism Spectrum Disorder (ASD).

  1. Request for excused absences for the treatment of ASD must be submitted on the District’s Request/Agreement for Excused Absences for the Treatment of Autism Spectrum Disorder (Form 5200.01 F1) and submitted a minimum of ten (10) school days prior to the start of treatment.
  2. Treatment must be provided by a licensed health care practitioner or Certified Behavior Analyst (CBA) pursuant to F.A.C 6A-1.09515.
  3. The request for excused absences for the treatment of ASD should be turned in to the school principal. A treatment plan, signed by the licensed health care practitioner or CBA providing the treatment, must accompany the request at the time of submission to the school principal. The licensed health care practitioner or CBA may not be the child’s parent/legal guardian.
  4. The Request/Agreement for Excused Absences for the Treatment of Autism Spectrum Disorder (Form 5200.01 F1) will not be applied retroactively and is only effective starting the date it is signed by the treating licensed health care practitioner or CBA and District administration.
  5. The Request/Agreement for Excused Absences for the Treatment of Autism Spectrum Disorder (Form 5200.01 F1) is valid only for the school year during which it is submitted.  A new Request/Agreement for Excused Absences for the Treatment of Autism Spectrum Disorder (Form 5200.01 F1) must be completed each subsequent school year thereafter.
